& Assistant Volunteer Coordinator Full Time
Position Minimum of Associates Degree or further education in marketing, or
relevant experience necessary.
Proficient skills in the use of Adobe Creative Suite, & Microsoft
Office Suite. Excellent command of
written language required. Knowledge
of medical terminology preferred. HKV offers

and salary requirements Patient Care Coordinator
We are
currently seeking a full time Patient Care Coordinator with a strong interest in hospice and palliative care, and a
holistic, patient/family focus. The Patient Care Coordinator is an RN
with Leadership skills that plans, directs and participates in the delivery
of hospice services. The successful candidate will be responsible for
the day-to-day operations of HKV including leading, educating and supervising
members from the hospice interdisciplinary team in an effort to meet the
goals of each patient’s plan of care. The Patient
Care Coordinator will possess proven leadership skills (as demonstrated by
successful direct supervision experience) excellent communication skills and
a desire to collaborate with our dedicated, interdisciplinary team.
Bachelor of Science in Nursing or advanced nursing degree required.
Certification as a CHPN strongly preferred; ELNEC a plus. Minimum of
three to five years experience as a Leader and/or Supervisor along with a
registered nurse with broad experience in the skilled nursing field
required. Community hospice or home health experience required.

Clinical responsibilities include education and
consultation for complex pain and symptom management, end-of-life decisions
and advance care planning. Our
patients are served in hospitals, homes and nursing homes. The Nurse Practitioner will collaborate
with the interdisciplinary team, including our full time Medical Director and
attending physicians to provide state of the art palliative and end of life
care for the residents of
